Facility registry
Every public and private facility with its ownership, level of care, type, services, location and GPS coordinates, searchable and mapped to state, LGA and ward.
HEFRIAS, the Health Facility Registration, Inspection & Accreditation System, gives ministries of health, regulators and partners one digital workflow. Register facilities, run standardised assessments in the field, score compliance automatically, cost every gap, and track the interventions that close it.
Illustrative sample data
Assessments shouldn't end as a report in a drawer. HEFRIAS connects what inspectors find to what it costs to fix, and to proof that it was fixed.
Forms differ between teams, results arrive weeks late, and evidence lives on personal phones.
Every inspector uses the same version of the same form, with photos and documents attached to the answer they support.
"Not available" tells planners what is wrong but not what it will cost to put right.
Each shortfall is matched to a standard procurement item with a low and high estimate, ready for budgeting and partner asks.
Donations and upgrades are hard to tie back to the facilities and gaps they were meant to address.
Interventions are logged against facilities, verified with evidence, and compared with the next round of assessment scores.
Six steps take a facility from the registry to a verified fix, and the next assessment round shows whether things got better.
Build one authoritative registry of public and private facilities, with ownership, level of care, facility type, services offered, LGA and ward, and GPS coordinates. Record the health workers at each site.
Start from a national module or design your own checklist. Group questions into thematic categories and choose how each is answered and scored.
Inspectors complete assessments on the web or the Android app. They attach photos and documents to individual answers, add comments, and log arrival and departure times. Facilities can also complete self-assessment forms.
Every submission is scored when it is saved. Results roll up by thematic area, facility, LGA and agency, so supervisors see where standards are slipping without waiting for a manual report.
A "No" answer, or a count below the required minimum, becomes a gap. HEFRIAS works out the shortfall and prices it against a standard procurement list, giving a low and high cost for each facility, LGA or state.
Log government and partner interventions against facilities, with donor, type, value and evidence. Verify or reject them, then compare scores before and after to see what changed.
Registration, assessment, analysis, accreditation and follow-up share one data model, so nothing has to be exported and re-entered between steps.
Every public and private facility with its ownership, level of care, type, services, location and GPS coordinates, searchable and mapped to state, LGA and ward.
A central record of health workers by facility, with NIN, job title and cadre, plus breakdowns by gender and by professional and support staff.
Manage accreditation status and renewals for each facility, and issue digital certificates once standards are met.
Design your own instruments with categories, questions, answer options, scores, minimum quantities and linked procurement items. Publish SOPs alongside them.
Health facility assessment modules, record reviews, exit interviews, case simulations and survey-monitoring tools, loaded and ready to use.
Web and Android data capture with photo and PDF evidence on each answer, comments, visit times, and facility self-assessment.
Option-based, quantity-gap and hybrid scoring, set per form. Scores are capped at 100% so oversupply in one area never hides gaps in another.
Missing and insufficient items are listed automatically, with the shortfall and a low and high cost estimate from a standard procurement catalogue.
Agency and facility dashboards with thematic compliance, trends, rankings and a summary of what inspectors' comments say most often.
Monthly, comparison and benchmark reports with a cover page and executive summary, exportable to Excel and PDF.
Record donor and government interventions by type and value, attach evidence, verify or reject them, and follow trends over time.
Access by role for each agency, a secure integration API for partner systems, and signed webhooks when new assessments come in.
Every form declares how it is scored, and every gap is costed by the same rule, so results are consistent from one inspector, facility and round to the next.
Each answer carries a value. The score is the points earned against the points possible.
Σ answer values ÷ Σ max scoresFor countable items such as beds, equipment or staff, the score is what's there against the required minimum, weighted by that minimum.
min(found ÷ required, 1)Mix both in one instrument. Where an item has a count and an answer, both must pass for full credit, for example "present" and "functional".
min(quantity score, answer score)A gap is a "No" answer or a count below the minimum. The shortfall is multiplied by the low and high price of the linked procurement item.
| Item | Required | Found | Short | Estimate |
|---|---|---|---|---|
| Delivery couch | 2 | 1 | 1 | ₦180k – ₦240k |
| Oxygen concentrator | 2 | 0 | 2 | ₦1.9M – ₦2.4M |
| Hospital beds | 20 | 14 | 6 | ₦1.1M – ₦1.4M |
| Autoclave ("No") | 1 | – | 1 | ₦650k – ₦820k |
Dashboards update as assessments come in. Reports are built from the same data, ready to present or export.
See compliance across the whole agency, then open any facility to see its own scores, gaps and history.
Results for any instrument, month by month, so progress across an assessment programme is easy to follow.
Put two facilities side by side, or the same facility across two assessment rounds, to see exactly where they differ.
Rank facilities against each other on the same instrument to find leaders to learn from and laggards to support.
See what has been invested, where, by whom, and whether it has been verified.
HEFRIAS runs in the browser and as an Android app, so assessment teams use the device they already carry, and every answer arrives with its evidence.
The same forms on desktop, tablet and phone, plus an installable web app.
Attach photos and PDF documents to the question they support.
Inspector notes are kept per question and summarised into recurring themes.
Assessment date and arrival and departure times are recorded with every inspection.
GPS coordinates and state, LGA and ward for every facility in the registry.
Facility teams sign in to complete forms marked as self-fillable and to report interventions.
Each role sees what it needs, from the state-wide picture to a single facility.
Run registration, inspection and accreditation programmes on standard instruments, with live oversight.
Know exactly which standards a facility is missing and what it will take to meet them.
Target support at documented, costed needs, and show verified results for what you fund.
Track quality of care across rounds and regions to plan budgets and measure what works.
Partner systems can fill HEFRIAS forms, read scores and costed gaps, and get notified of new assessments, all under your agency's control.
Approved systems list forms, register facilities (duplicates are matched automatically), submit completed assessments and read results, gaps and costs.
Partner systems are notified when a new inspection is submitted. Each message is signed with HMAC-SHA256 so the receiver can confirm it came from HEFRIAS.
The shared procurement catalogue lets partner marketplaces turn costed gaps into offers from vendors.
// Sent when an inspection is submitted { "event": "inspection.submitted", "inspection_id": 51, "agency_id": 1, "facility_id": 12, "inspect_form_id": 3, "source": "hefrias" } // Then fetch scores and costed gaps GET /api/integration/v1/submissions/51 { "metrics": { "percentile": 72.4 }, "gap_totals": { "low_cost": 3820000, "high_cost": 4860000 } }
National health facility assessment modules and survey tools are already built in, with categories, questions and answer options configured.

Yes. The checklist builder lets you create instruments with your own categories, questions and answer types, including yes/no, multiple choice, counts and free text. You also set the scores, minimum quantities and linked procurement items. The national modules can be used as they are or as a starting point.
Each instrument uses one of three methods. Option-based scoring adds up answer values. Quantity-gap scoring compares counts with required minimums. Hybrid scoring combines the two, so an item is fully compliant only when both its count and its answer pass. Scores are capped at 100%, so oversupply of one item can't hide a gap in another.
Question items can be linked to a procurement catalogue that holds a low and a high unit price. When an assessment finds a gap, HEFRIAS multiplies the shortfall by those prices. You manage the catalogue, so estimates reflect your local market.
Yes. HEFRIAS runs in any modern browser and as an Android app, and inspectors can attach photos and PDF documents to individual answers.
Yes. Facility staff have their own sign-in and can complete any instrument marked as self-fillable. They can also report interventions, which the agency then verifies.
Yes. A secure integration API lets approved systems read forms, register facilities, submit assessments and fetch scores and costed gaps. Signed webhooks notify them when new inspections are submitted.
Data is scoped to each agency, and access depends on role: agency administrators, inspection staff and facility personnel each see only what their role allows. Integration keys are stored hashed and are limited to the agencies they were issued for.
We'll show you the platform with your instruments, your facilities and your reporting needs.
